 Email your request to us : email us with your request, remembering to tell us which Surgery you collect your medication from (if a dispensing patient) or, if you are a non-dispensing patient, where you collect your FP10 green prescription form from.
PLEASE REMEMBER THAT EMAIL IS NOT A SECURE MEDIUM AND YOU MAY BE AT RISK OF YOUR CONFIDENTIALITY BEING BREACHED. The email prescription service will send you back an "autoresponse" to acknowledge that we have received your request. If you share a computer with others in your household, they may therefore become aware that you have ordered medication. Please be aware of this when using the service.
For dispensing patients, we do encourage you to collect your medication from Churchill surgery, irrespective of whether you normally see your doctor there or not : this is because the dispensing team work at Churchill and so are on hand to answer any queries you may have when you collect your medication. If you wish to change your collection point from Wrington to Churchill surgery, please let us know so we can amend your records accordingly.
To register for the email prescription service, ask us for a registration form or download it from here, complete and sign it and drop it back into either of the Surgeries.
Use the prescription counterfoil. Simply tick the items you require and either
Drop the counterfoil in when you are collecting your monthly repeat order and state the approximate date you would like to collect the next supply
Post the counterfoil to us
Drop it into the repeat prescriptions box at either surgery or post it through the letterbox
Fax your request to the relevant surgery (Churchill 01934 852551, Wrington 01934 863568)
We are unable to take requests for repeat prescriptions over the telephone.
